ORENCIA (abatacept) exerts its therapeutic effects through selective modulation of T cell costimulation, specifically by inhibiting the CD80/CD86:CD28 interaction necessary for full T lymphocyte activation. This immunomodulatory action underlies its efficacy in reducing disease activity in autoimmune conditions such as moderately to severely active rheumatoid arthritis (RA), polyarticular juvenile idiopathic arthritis (pJIA), and psoriatic arthritis (PsA) in both adults and pediatric patients aged ≥2 years. Additionally, it is employed for prophylaxis of acute graft-versus-host disease (aGVHD) in hematopoietic stem cell transplant recipients. Its use leads to decreased inflammatory cytokine release, lowered immune cell proliferation, and clinical improvement in joint function and systemic symptoms. Its benefit-risk profile includes infection risk and is contraindicated in concurrent use with other biologic immunosuppressants.
