Usage and dosage of Agamree (vamorolone)
1 Evaluation before the first medication
Before starting AGAMREE treatment, all necessary immunizations should be completed in accordance with the immunization guidelines. For live vaccines or live attenuated vaccines, they should be completed at least 4 to 6 weeks before starting AGAMREE.
2 Dosing information
The recommended regular dose is 6 mg/kg orally once a day, preferably with meals; for patients weighing more than 50 kg, the maximum daily dose is 300 mg. Some patients may respond to a daily dose of 2 mg/kg, and the dose can be reduced to 2 mg/kg/day based on individual tolerance.
3 Recommended dose for patients with impaired hepatic function
For patients with mild (Child-Pugh A) to moderate (Child-Pugh B) hepatic impairment, the recommended dose is 2 mg/kg orally once daily, preferably with meals; for patients weighing more than 50kg, the maximum daily dose is 100mg.
The dose may be adjusted downward as appropriate based on individual tolerance.
4 Important preparation and administration procedures
AGAMREE oral suspension should be shaken for approximately 30 seconds before administration.
Please only use the oral syringe provided with the product: draw the appropriate amount into the syringe and deliver the solution directly into the mouth.
Any remaining solution after opening the bottle should be discarded if not used within 3 months.
5 Switching from other glucocorticoids to AGAMREE
Patients can switch from oral glucocorticoids (such as prednisone or prednisolone) to AGAMREE without interruption, without first reducing the glucocorticoid dose, to reduce the risk of adrenal insufficiency. For patients who have been receiving oral glucocorticoids for a long time, the switch can start from 6 mg/kg per day.
6 Dose adjustment when used with strong CYP3A4 inhibitors
When used with strong CYP3A4 inhibitors (such as itraconazole), the recommended dose is adjusted to 4 mg/kg orally once a day, preferably with meals; for patients weighing more than 50 kg, the maximum daily dose is 200 mg. The dose can be appropriately lowered according to individual tolerance.
7 Dose reduction when stopping the drug
If the drug is used continuously for more than one week, the dose of AGAMREE must be gradually reduced to reduce the risk of adrenal insufficiency caused by sudden cessation.
