Individualized Titration for Optimal Tolerability
Exelon (rivastigmine tartrate) should be administered with meals in divided doses, morning and evening. For Alzheimer’s disease, the initial dose is 1.5 mg twice daily, increasing to 3 mg, 4.5 mg, and 6 mg twice daily at minimum two-week intervals if tolerated, with a maximum of 6 mg twice daily. For Parkinson’s disease dementia, the same titration applies, but each step requires a minimum of four weeks. Dose adjustments are necessary for patients with renal or hepatic impairment, or low body weight, as they may only tolerate lower doses. If treatment is interrupted for more than three days, reinitiation should start at the lowest dose.
