Defitelio (defibrotide sodium) is to be administered intravenously at a dose of 6.25 mg/kg every 6 hours, delivered over a 2-hour infusion period. This regimen is recommended for a minimum duration of 21 days. If signs and symptoms of hepatic veno-occlusive disease (VOD) have not resolved after this initial period, treatment should continue until resolution is achieved.
