VYALEV Dosing and Administration Protocol
The administration of VYALEV (foscarbidopa and foslevodopa) is exclusively subcutaneous, utilizing the VYAFUSER pump. The base continuous dosage and hourly infusion rate are calculated based on the patient's total daily oral levodopa (TLD) intake, converted to a foslevodopa equivalent by multiplying the TLD by 1.3, then dividing by 240 (mg of foslevodopa per mL) to get the total daily volume, which is subsequently divided by the patient's typical waking hours (e.g., 16 hours) or 24 hours for continuous infusion. An optional loading dose, calculated from the first morning oral immediate-release levodopa dose (multiplied by 1.3 and divided by 240 for VYALEV dose in mL), can be administered if initiating in an "Off" state or after an infusion interruption exceeding 3 hours. Extra doses (0.1 mL to 0.3 mL, equivalent to approximately 17 mg to 51 mg levodopa) can be programmed, limited to one per hour. The maximum recommended daily dosage is 3,525 mg of foslevodopa.
