When co-administered with strong CYP3A4 inhibitors (e.g., itraconazole), dosage is adjusted to 4 mg/kg (max 200 mg/day). Abrupt discontinuation after >1 week of use is contraindicated; gradual tapering is required to mitigate adrenal insufficiency.
